CLAYMAN THYROID CENTER ADVANCE BENEFICIARY NOTICE
And Notification of Research Policies
This page outlines the billing policy of the Clayman Thyroid Center. All patients are required to acknowledge their understanding of this policy prior to undergoing consultation with us.
There are three steps in our process which we'll outline below: Consultation, Evaluation, and Surgery.
1. Consultation With an Expert Surgeon
Whether you choose an in-person consultation or a virtual phone consultation, there is no separate fee for this service. It is included as part of your care at the Clayman Thyroid Center.
During your consultation, one of our expert thyroid surgeons will thoroughly review your medical records and imaging ahead of time. They will then meet with you to discuss your diagnosis, answer questions, and outline the recommended next steps.
Your insurance may still apply deductibles or co-pays for in-person visits, which our team will help you understand ahead of time. For patients traveling from outside Florida, a virtual second opinion consultation allows us to guide you efficiently without requiring multiple trips to Tampa.
2. Evaluation at the Hospital
When you arrive in Tampa for your evaluation, we will perform a high-resolution ultrasound and any additional diagnostic tests required to finalize your treatment plan. This step helps us determine whether surgery is needed and what procedure would be best.
The cost of this evaluation is billed to your insurance. Most major health plans—including Medicare, BC/BS, Humana, Aetna, Cigna, and UnitedHealthcare—cover this evaluation as part of specialist care. Our team will verify coverage and discuss any deductibles or co-pays prior to your visit.
3. The Thyroid Operation
We are in-network for most major health plans and many smaller ones. This means the surgery itself will be covered by almost all insurance companies. The exact amount covered depends on your plan, deductibles, and other details, but all major plans include thyroid surgery as a covered benefit.
For patients without insurance, we offer affordable out-of-pocket pricing. Our self-pay package includes the consultation, evaluation, surgeon’s fee, anesthesia, operating room, hospital charges, and post-operative care—everything except travel and lodging. To discuss pricing, email our billing manager at linda@parathyroid.com
Our billing team will provide a detailed breakdown of your coverage and any patient responsibility prior to your surgery date. Payment of co-pays and deductibles is due at the time of service. If needed, payment plans can be arranged—please contact Nicole via email or by phone at (813) 940-3130 for assistance.
Appeals
All patients have the right to appeal any decision made by their insurance or Medicare carrier regarding payments or services. This is federal law.
Questions Regarding Your Bill
Questions should be addressed to Nicole via email or by phone at (813) 940-3130. Please do not contact her until after your consultation has taken place.
Ongoing Research
The physicians at the Clayman Thyroid Center are actively engaged in research to improve diagnosis and treatment of thyroid disease. By becoming a patient, you acknowledge that your medical records may be included—anonymously and in aggregate—with hundreds or thousands of others for research purposes.
Your privacy is paramount. No information will be used in a way that could personally identify you. You may opt out of research at any time by emailing Dr. Clayman.
